Maybe not checking all the boxes, but I recently got a Petzl Tikka (CORE li-ion version), and for a small budget headlamp, it's fine. One 5mm red LED in addition to the single white LED (350-450lm). The red is either on (2lm) or flashing, though. 3 AAA batts or Petzl CORE li-ion pack.

I own a Petzl Tactikka +RGB 350lm. There's a lot to be said for simplicity of the UI. The red/grn/blue is only one brightness level. Enough lumens to see in front of your feet, and dig for treasures at the beach at night.
Petzl headlamps are fairly inexpensive if you don't add their lion battery packs, they're on the pricey side.

Feeling a bit silly as I didn't realize it has a red LED option on it.
(Just wanted a very compact but tough headlamp.)
Black Diamond Spotlight160 headlamp.
Only had it a short time. So far, so good.
But the U.I. is a bit of a pain in the neck.

If you are still looking, this might fit the bill (and is very inexpensive): Armytek Crystal Headlamp review I just posted.

It's not pretty to look at, but it has multiple levels for both the red and white LEDs. I found the red LED surprisingly light for such a budget light.

And the built-in pocket clip makes a great option if you want to mount it on a belt on jacket pocket instead.
